For the times when students feel under the weather while at school, you can rest assured that our School Nurse and Health Clinic Staff are here to help. In order to provide the proper care, we ask parents to download and complete the following forms and return them to your child's teacher at the beginning of the school year. You may also be asked to fill out additional forms.

South Dearborn Community School students also have access to a multitude of health professionals and services, including:

VACCINATIONS/IMMUNIZATIONS


In order to keep all of our students healthy, we request that certain immunications be administered. You can review the School Immunization Requirements list from the Indiana State Department of Health here.

then visit MyVaxIndiana.in.gov and use your PIN# for access.

Indiana State Law IC 20-30-5-18 requires that school systems provide important information to parents and guardians of all students about meningitis and the vaccines available to prevent one type of this serious illness at the beginning of each school year.
